Thank you for applying to be a Boost client. To ensure we can match you appropriately, and run the programme effectively please provide the following personal, employment and equality monitoring details.

DATA PROTECTION STATEMENT

You have the right to know how we use your personal information. It is important that you read the information below.

All volunteers on Business in the Community’s Boost programme should be aware that by providing the information contained within this form, you consent to Business in the Community holding this information confidentially for the purpose of running the programme and evaluating its effectiveness.

For research and monitoring purposes, your individually identifiable data will be held on our system for a period of 6 months following the end of the programme after which time all personal data will be deleted. When sharing positive outcomes and case studies, we will always anonymise who it belongs to, unless we gain your permission to use your details. At any point during or following your involvement with Boost , you have the right to request information we hold about you, we call this Right to Access. All requests will be dealt by the Programmes Team alongside the National Data Protection Officer.
